Creating a custom waiver template
Create Your Own is the Waiverly template for businesses that don't fit one of the built-in industries we have listed on the app: an event organizer, a photography studio, a dog groomer, a pottery class, anything that needs a signed waiver. You write (or AI-draft) your own content, decide which questions and client details to collect, and optionally add fields for your staff to complete. Form links, ID photos and custom sections work the same as in the industry-specific templates.
This guide walks through all eight steps of the builder, then shows what your client sees.
In short: Create Template → Create Your Own → write your waiver sections → choose settings → add any staff fields → set your liability clauses → add health questions → adjust the client information fields → confirm the legal details → preview and save.
Open the menu (☰), tap Create Template, choose the Create Your Own tile and tap Let's get started.
Step 1: Waiver Content
- Enter a Waiver Name.
- Choose the Waiver Language (English, Spanish or French). It sets the language of the generated text and the PDF labels and is locked after creation, so pick carefully.
- Fill in Country, State and City/Local Jurisdiction.
- Under Create custom sections, enter a Section title and the Section content. If you'd rather start from a draft, tap the sparkle (AI) button and edit what it writes.
- Tap Add Section. It appears under Sections in your waiver (for example "Waiver Terms"). Add as many sections as you need and drag them to reorder; that's the order used in the PDF.
- Tap Next.
Screen needed: Step 1 Waiver Content, phone layout, with the Waiver Name filled in (e.g. "Climbing Wall Waiver"), a section called "Waiver Terms" already in the Sections in your waiver list, and the Create custom sections box above it. Show: the sparkle (AI) button next to the Section content field.
Step 2: Waiver Settings
| Setting | What it does |
|---|---|
| Require ID Photo | Adds a Client ID Capture step. Turning it on shows an I understand checkbox: "Be sure to consult your legal advisor to ensure compliance with state laws before including this feature." |
| Email Waiver Submissions via Waiverly | Emails the signed waiver to the client once the submission is completed. |
| Accept Form Link Submissions | Lets clients fill out and sign the waiver through the template's form link. |
| Age Restriction | No Age Restriction, or Minimum Age Requirement with the age you enter (for example 18). |
| Custom Clauses | Adds a step where the client initials each clause from Step 4. |
| How You Found Us | Asks the client how they heard about you, which feeds the Client Acquisition Sources chart in Analytics. |
| Health Background | Includes "a customizable health section where clients answer the questions you define for this waiver" (shaped in Step 5). |
Step 3: Staff Entry
The Staff Entry Editor is for "optional staff-completed fields you want your team to fill in during the waiver flow", such as the service booked, the instructor, or an internal reference. Clients never see these. "Leave this empty if no extra business details are needed."
- Tap Add Field. The Add business field sheet opens: "Set the field label, answer type, and whether staff must complete it."
- Enter the Field label and choose the type: Text Field, Number Field, Date Picker, Dropdown or Multiselection.
- Tick Require an answer before staff can continue if the field must be filled in, then tap Save.
- Repeat for each field, then tap Next.
Below the fields is the Skip Submission Review toggle: "Allow clients to complete and sign this form without needing to review the submission on the "Form Links" dashboard." It's available only while there are no staff fields. With it on, a form-link client signs in one go and the submission is completed immediately; with it off, or with staff fields present, each form-link submission waits for your staff inputs or confirmation under Business Action.
Screen needed: Step 3 Staff Entry Editor, phone layout, with the Add business field sheet open, "Instructor" typed as the field label, Dropdown chosen as the type and the Require an answer checkbox ticked. Show: the five answer types and the required checkbox.
Step 4: Liability Clauses
- Type a clause in the clause box, or tap the AI button for a draft, then tap Add Clause.
- Drag the clauses in the list to set their order. If Custom Clauses is on, the client initials each one in this order.
- Tap Next.
Step 5: Health Questions
The Health Editor lets you "review the default health background blocks, remove the ones you do not need, and add your own custom questions". This step only matters when Health Background is on in Step 2.
- Three default blocks are included: Allergies, Health Conditions and Diseases. Each is a Multiselection, optional, with an additional-details box, and is marked Default block. Tap the pencil to edit one or the trash icon to remove it.
- Tap Add Question to add your own question.
- Drag the cards to set the order; this is the order clients see in Health Background and in the PDF.
- Tap Next.
Screen needed: Step 5 Health Editor, phone layout, showing the Allergies, Health Conditions and Diseases cards with their "Multiselection · Optional · Additional details · Default block" chips and the Add Question button. Show: the pencil and trash icons on a default block.
Step 6: Client Information
The Client Information Editor decides what you ask every client. The core identity fields are fixed; everything else can be edited, removed, reordered or shown only when a condition is met.
- The fields under Always included · Preview only can't be changed: First Name (Text Field), Last Name, Birthday (Date Picker) and Email.
- Below them are the default fields, for example Gender (Dropdown, Optional, with Male / Female / N/A), Phone Number (Number Field, Required), Address, Emergency Contact Name and Emergency Contact Phone Number. Each has a logic icon, a pencil and a trash icon. Remove any you don't need.
- Tap Add Field to add your own input, such as "Membership number" or "Shoe size".
- To show or hide a field based on another answer, tap its logic icon. The Logic Rules sheet ("Show or hide this field based on conditions.") asks you to choose Show this field or Hide this field, pick the field under If field…, choose a Condition… and enter a Value…, then tap Save Rule. Delete removes a rule.
- Drag the fields into the order you want, then tap Next.
The conditions available are: is exactly, is not, is empty / blank, is not empty, contains, does not contain, is less than, is more than, age under and age over. The last two work from the Birthday field, so you can, for example, show a "Parent or guardian name" field only when the client's age is under 18.
Screen needed: Step 6 Client Information Editor, phone layout, with the Logic Rules sheet open for a field called "Parent or guardian name": Show this field selected, If field set to Birthday, Condition set to "age under", Value 18. Show: the Condition dropdown with "age under" chosen.
Step 7: Jurisdiction
- Optionally, enter a law or regulation web address under Add Law URL (up to 250 characters) and tap +.
- Add any Additional Legal Notes (up to 300 characters).
- Tick I confirm that the above information is accurate and understand its legal implications. and tap Save & Proceed.
Step 8: Preview
The last step shows the finished waiver as a PDF. Page through it, go back to any step if something needs changing, and tap the button at the bottom to save. The template then appears on the Waiver Forms screen.
What your client sees
Whether the client signs on a device in your business or through a form link, they first see a PDF Preview of the waiver, then the form takes them through eight steps:
- Client Information: the fixed identity fields plus the fields you kept or added in Step 6, with any logic rules applied.
- Client ID Capture, if Require ID Photo is on.
- Health Background: your health blocks (for example Allergies, Health Conditions and Diseases) as expanders, each with an "Additional details (optional)" box.
- Custom Clauses: the client types their initials under each clause. The initials must match the client's first and last name; otherwise the form shows "Initials do not match first and last name".
- How You Found Us, if enabled.
- PDF Preview of everything they entered. With Skip Submission Review on, the buttons are Cancel Submission and Proceed to Sign. With it off, or when the template has staff fields, the client taps Submit for Review instead and the submission waits for your team on the Form Links screen.
- Signature: the client signs on screen.
- Waiver Complete: the signed waiver is done.
Tapping Cancel Submission asks "Confirm Cancellation – Are you sure you want to cancel this submission? All inputted data will be lost." with No / Yes.
Screen needed: the client-side PDF Preview step of a custom form link on a phone, with the step list showing step 6 of 8, the filled-in waiver above and the Cancel Submission and Proceed to Sign buttons below. Show: Proceed to Sign, which only appears when Skip Submission Review is on.
Good to know
- Language is permanent. In Edit waiver the Waiver Language is locked; create a separate template for each language you want to offer.
- Staff fields and Skip Submission Review are either/or. As soon as you add a business field, Skip Submission Review becomes unavailable and every form-link submission needs a review.
- Core fields can't be removed. First Name, Last Name, Birthday and Email are always collected, even if you delete every other client field.
- Health answers stay private. What clients enter under Health Background is kept on the submission and never used in Analytics.
