Creating a PMU waiver template

The PMU template is Waiverly's ready-made waiver for permanent makeup studios: brows, eyeliner, lips, scalp micropigmentation, areola work and more. You work through a step-by-step builder, save your procedure and relevant options, and the waiver is ready to sign in your studio or through a form link.

This guide covers each step of the builder, then what your client sees and where the artist's part comes in.

In short: Create Template → PMU → name the waiver, set its language and jurisdiction, add your artists → choose your settings → save your procedure options → tweak the sections → confirm the legal details → check the preview and save.

Before you start

Have these ready and the builder takes a few minutes:

  • Your artists' names and license numbers. A license number is required for each artist; type "n/a" if your jurisdiction doesn't issue or require one.
  • Your jurisdiction: the country, state and city or local jurisdiction where the studio operates.
  • The waiver language (English, Spanish or French). It's set once, at creation, and can't be changed later. If you need to support several language options you will need to create a separate waiver template for each case.
  • Your procedure options: the treatment areas, procedure types, devices, needle configurations, pigment brands and pigment colors you use. Defaults are included, so you only add what's missing.

Open the menu (☰), tap Create Template, choose the PMU tile and tap Let's get started.

Step 1: PMU Artists

  1. Enter a Waiver Name (up to 50 characters).
  2. In the Waiver language card, tap Selected waiver language, choose English, Spanish or French and tap Apply. This decides the language of the AI-generated text, the acceptance statement defaults and the PDF labels, and it can only be set during creation.
  3. Fill in Country, State and City/Local Jurisdiction.
  4. Enter each artist's Artist Name and License Number and tap the add button. The license is required; if no license number exists, type "n/a". Added artists appear in a list with their license and a trash icon.
  5. Tap Next. It stays disabled until the required fields are filled in.

Screen needed: Step 1 PMU Artists, phone layout, with the Waiver Name filled in (e.g. "Brow Studio PMU Waiver"), the Waiver language card showing English, and one artist already in the list (e.g. "Lena Park · License PMU-8832") above the empty Add artist fields. Show: the "Set this during creation only" note under the selected waiver language.

Step 2: Waiver Settings

The Compliance Settings card lets you configure common compliance requirements for your PMU waiver. Each toggle adds or removes a part of the waiver.

Setting What it does
Require ID Photo Adds a Client ID Capture step where the client photographs or uploads their ID.
PMU Details Adds key details about the PMU session, including the artist, date, treatment area, procedure, pigments and session notes. The artist completes this part after the client submits, using the options you save in Step 3.
Medical History Includes a health questionnaire for the client to answer.
Sterilization Details Adds a section where the artist records sterilization information for the session.
Military Info Adds military-related information to the client's intake.
Special Instructions Adds a section for the artist's instructions or notes for the client.
Supporting Documents Lets the client upload supporting files with their submission.
Acceptance Statement Includes the statement clients acknowledge before signing (edited in Step 6).
Email Waiver Submissions via Waiverly Emails the signed waiver to the client once the submission is completed.
Accept Form Link Submissions Lets clients fill out and sign the waiver through the template's form link.
Skip Submission Review Lets a form-link client complete and sign in one go. It's greyed out while PMU Details is on, because the artist needs to record the session first.
Age Restriction No Age Restriction, or Minimum Age Requirement with the age you enter.
Media Consent Asks the client for permission to use photos of their results.
Custom Clauses Adds a step where the client initials each of your liability clauses.
How You Found Us Asks the client how they heard about you, which feeds the Client Acquisition Sources chart in Analytics.

Screen needed: Step 2 Waiver Settings, phone layout, scrolled so that PMU Details (on), Accept Form Link Submissions (on) and Skip Submission Review (greyed out) are visible together. Show: Skip Submission Review greyed out beneath the enabled PMU Details toggle.

Step 3: Procedure Options

The PMU Procedure Options step is where you "add the treatment areas, procedure types, device types, needle configurations, pigment brands, and pigment colors your studio commonly uses". These saved options are what the artist picks from when recording a session, and they're the names that appear in your PMU Overview analytics. Each section has a text field, an Add button and a row of chips you can remove.

  1. Treatment Areas: Brows, Eyeliner, Lips, Lash Line, Scalp, Areola and Beauty Mark are included by default.
  2. Procedure Types: the techniques you offer, for example Classic Eyeliner, Lash Enhancement and Scalp Micropigmentation.
  3. Device Types: Rotary PMU Machine, Digital PMU Pen, Wireless PMU Machine, Hand Tool.
  4. Needle Configurations: 1RL, 3RL, 5RL, 1RS, 3RS, 5RS, 3F, 5F, U Needle, Nano.
  5. Pigment Brands: Perma Blend, Tina Davies, LI Pigments, Evenflo, Hanafy, Brow Daddy.
  6. Pigment Colors: Black, Dark Brown, Medium Brown, Light Brown, Warm Blonde, Ash Brown, Pink Nude, Rose, Red Modifier, Orange Corrector.
  7. Add anything missing with the field and Add button, remove the chips you don't use, then tap Next.

Screen needed: Step 3 PMU Procedure Options, phone layout, showing the intro notice and the Treatment Areas and Procedure Types sections with their default chips, with "Powder Brows" just typed into the Procedure Types field. Show: the add field with its Add button above the row of chips.

Tip: keep one spelling per option. If "Dark Brown" and "Dk Brown" are both saved, they count as two different colors in Analytics.

Step 4: Customization

This step holds the written sections of the waiver, in the order they'll appear in the PDF.

  1. Under Create custom sections, enter a Section title and Section content, or tap the sparkle (AI) button for a draft you can edit.
  2. Tap Add Section. It appears under Sections in your waiver.
  3. Drag the sections to reorder them; this is the order used in the PDF. Tap Next.

Step 5: Liability Clauses

With Custom Clauses toggled on in Waiver Settings, the client is required to enter their initials on each clause when they fill in the waiver, signifying their consent.

  1. Review the list under Clauses in your waiver and remove anything that doesn't apply.
  2. To add a clause, type it in Enter Clause (Optional) (up to 1,000 characters) or tap the AI button, then tap Add Clause.
  3. Drag the clauses into order and tap Next.

Screen needed: Step 5 Liability Clauses, phone layout, with the Create clause box at the top and the first three default clauses underneath with their drag handles. Show: the "Drag to reorder." note above the clause list.

Step 6: Acceptance Statement

Here you "define the Acceptance Statement your clients will acknowledge and sign". A default statement is provided in the text field. Edit it directly or tap the AI button to generate a fresh version, then tap Next.

Step 7: Jurisdiction

The Jurisdiction & Legal Compliance step records the legal references behind your waiver.

  1. Optionally, enter a law or regulation web address under Add Law URL (up to 250 characters) and tap +.
  2. Add any Additional Legal Notes (up to 300 characters).
  3. Tick I confirm that the above information is accurate and understand its legal implications.
  4. Tap Save & Proceed.

Screen needed: Step 7 Jurisdiction & Legal Compliance, phone layout, with one law URL added, a short note in Additional Legal Notes and the confirmation checkbox ticked. Show: the Save & Proceed button at the bottom.

Step 8: Preview

The final step shows the generated waiver as a PDF. Page through it, go back to any step if something needs changing, and tap the button at the bottom to save. The template then appears on the Waiver Forms screen with a PMU category chip.

What your client sees

In the app, tap the template card on Waiver Forms, choose New Client or Return Client and hand over the device. Through a form link, the client opens the link on their own phone. They start with a PDF Preview of the waiver, then move through the client steps you enabled: Client Information, Client ID Capture, Supporting Documents, Medical History, Custom Clauses, Media Consent and How You Found Us, ending with a PDF Preview of what they entered.

When PMU Details is on, a form-link client taps Submit for Review rather than signing on the spot, as the artist has not entered their details. The submission waits on your Form Links screen. When your team taps Proceed, a  screen lists the artist requirements, based on what you've enabled. The artist, records the treatment, procedure, device, needles and pigments from the options in Step 3, and notifies the client to sign.

Good to know

  • Language is permanent. In Edit waiver the Waiver Language card shows a lock icon and reads "This waiver language is locked because it can only be set during creation." Make a separate template per language.
  • Editing later: on Waiver Forms, tap ⋯ → Edit waiver and confirm. The form link keeps working and shows the updated waiver.
  • Skip Submission Review is only available with PMU Details off, which also removes the artist's session record and the data behind PMU Overview analytics. This features allows clients to remotely sign a waiver without having staff review their submission.
