Creating a piercing waiver template
The Piercing template is Waiverly's ready-made waiver for piercing studios. You work through a step-by-step builder, switch on what you need, add your jewelry options, and the waiver is ready to sign on a device in your studio or through a form link.
This guide walks through every step of the builder, then shows what your client sees and where the piercer's part comes in.
In short: Create Template → Piercing → name the waiver, set its language and jurisdiction, add your piercers → choose your settings → save your jewelry options → tweak the sections → confirm the legal details → check the preview and save.
Before you start
The builder is quicker if you have these to hand:
- Your piercers' names and license numbers. Every artist entry needs a license number; if your jurisdiction doesn't issue or require one, you'll type "n/a".
- Your jurisdiction: the country, state and city or local jurisdiction where the studio operates.
- The language you want the waiver in (English, Spanish or French). This can't be changed after the waiver is created. If you want to support multiple languages you will have to create separate waivers for each language.
- Your jewelry options: the brands, types, gauges, sizes and materials you commonly fit. Industry-standard defaults are included, so you only need to add what's missing.
Open the menu (☰), tap Create Template, choose the Piercing tile and tap Let's get started.
Step 1: Piercing Artists
- Enter a Waiver Name (up to 50 characters). This is the name you'll see on the Waiver Forms screen.
- In the Waiver language card, tap Selected waiver language, pick English, Spanish or French and tap Apply. This sets the language of the AI-generated text, the acceptance statement defaults and the PDF labels. You can set this during creation only; it's locked afterwards.
- Fill in Country, State and City/Local Jurisdiction.
- Under Add piercing artist, enter the Artist Name and License Number, then tap Add Piercing Artist. The license is required; if no license number exists, type "n/a". Repeat for each piercer. Each one appears in the list below with their license and a trash icon to remove them.
- Tap Next. The button stays disabled until the required fields are filled in.
Screen needed: Step 1 Piercing Artists, phone layout, with the Waiver Name filled in (e.g. "Studio Piercing Waiver"), the Waiver language card showing English, and one artist already added in the list (e.g. "Sam Rivera · License PC-2041") above the empty Add piercing artist fields. Show: the "Set this during creation only" note under the selected waiver language.
Step 2: Waiver Settings
The Compliance Settings card lets you "configure common compliance requirements for your piercing waiver". Tap Show More to reveal the rest of the list. Each setting adds or removes a part of the waiver; switch on only what your studio uses.
| Setting | What it does |
|---|---|
| Require ID Photo | Adds a Client ID Capture step where the client photographs or uploads their ID. |
| Piercing Details | Adds key details about the piercing, including the artist, date, placement, jewelry and procedure notes. The piercer completes this part after the client submits, using the options you save in Step 3. |
| Medical History | Includes a health questionnaire for the client to answer. |
| Sterilization Details | Adds a section where the piercer records sterilization information for the session. |
| Military Info | Adds military-related information to the client's intake. |
| Special Instructions | Adds a section for the piercer's instructions or notes for the client. |
| Supporting Documents | Lets the client upload supporting files with their submission. |
| Acceptance Statement | Includes the statement clients acknowledge before signing (edited in Step 6). |
| Email Waiver Submissions via Waiverly | Emails the signed waiver to the client once the submission is completed. |
| Accept Form Link Submissions | Lets clients fill out and sign the waiver through the template's form link. |
| Skip Submission Review | Lets a form-link client complete and sign in one go. It's greyed out while Piercing Details is on: "Submission review is required while Piercing Details is enabled." |
| Age Restriction | No Age Restriction, or Minimum Age Requirement with the age you enter. |
| Media Consent | Asks the client for permission to use photos of their piercing. |
| Custom Clauses | Adds a step where the client initials each of your liability clauses. |
| How You Found Us | Asks the client how they heard about you, which feeds the Client Acquisition Sources chart in Analytics. |
Screen needed: Step 2 Waiver Settings, phone layout, scrolled so that Piercing Details (on), Accept Form Link Submissions (on) and Skip Submission Review (greyed out) are all visible. Show: the grey "Submission review is required while Piercing Details is enabled." note under Skip Submission Review.
Step 3: Piercing Details
Here you "add the jewelry brands, types, gauges, sizes, and materials your studio commonly uses". These saved options are what the piercer picks from when they record what was used during a session, and they're also the names that appear in your Piercing Overview analytics. Industry-standard defaults are already included.
- Scroll through the five sections: Jewelry Brands (Anatometal, Industrial Strength, NeoMetal, BVLA, Junipurr), Jewelry Types (Labret Stud, Straight Barbell, Curved Barbell, Circular Barbell, Captive Bead Ring, Seam Ring, Clicker), Gauge Options, Size Options (6 mm, 8 mm, 10 mm, 12 mm, 3/8 in) and Material Options (Implant-Grade Titanium, Surgical Steel, 14K Gold, Niobium, PTFE, Glass).
- To add an option, type it in the field at the top of the section and tap Add. Remove any chip you don't use so the piercer's lists stay short.
- Tap Next.
Screen needed: Step 3 Piercing Details, phone layout, showing the intro notice and the Jewelry Brands and Jewelry Types sections with their default chips, with "Junipurr" just typed into the Jewelry Brands field. Show: the add field with its Add button above the row of chips.
Tip: use one spelling per option. "Implant-Grade Titanium" and "Titanium" saved side by side will show up as two separate entries in Analytics.
Step 4: Customization
This step holds the written sections of your waiver, in the order they'll appear in the PDF.
- Under Create custom sections, enter a Section title and the Section content. To have Waiverly write a first draft, tap the sparkle (AI) button and edit the result.
- Tap Add Section. It joins the list under Sections in your waiver (for example "Piercing Waiver Terms").
- Drag the sections to reorder them. "This is the order used in the PDF."
- Tap Next.
Step 5: Liability Clauses
The Custom Liability Clauses step comes with a set of default clauses for piercing, such as confirming the client is 18 or older, agreeing to disclose health conditions, acknowledging anatomical concerns and accepting the inherent risks of the procedure. If Custom Clauses is on in Waiver Settings, the client has to write down their initials for each clause when they fill in the waiver, signifying their consent.
- Review the list under Clauses in your waiver and remove any clause that doesn't apply to your studio.
- To add one, type it in Enter Clause (Optional) (up to 1,000 characters) or tap the AI button for a draft, then tap Add Clause.
- Drag the clauses into the order you want, then tap Next.
Screen needed: Step 5 Liability Clauses, phone layout, with the Create clause box at the top and the first three default clauses visible underneath with their drag handles. Show: the "Drag to reorder." note above the clause list.
Step 6: Acceptance Statement
Here you "define the Acceptance Statement your clients will acknowledge and sign". A default statement is provided in the waiver's language. Edit the text directly, or tap the AI button to generate a new version, then tap Next.
Step 7: Jurisdiction
The Jurisdiction & Legal Compliance step records the legal references behind your waiver.
- Optionally, enter a law or regulation web address under Add Law URL (up to 250 characters) and tap +. You can add more than one.
- Add any Additional Legal Notes (up to 300 characters).
- Tick I confirm that the above information is accurate and understand its legal implications.
- Tap Save & Proceed.
Screen needed: Step 7 Jurisdiction & Legal Compliance, phone layout, with one law URL added, a short note in Additional Legal Notes and the confirmation checkbox ticked. Show: the Save & Proceed button at the bottom.
Step 8: Preview
The last step shows the generated waiver as a PDF. Page through it, go back to any step if something needs changing, and tap the button at the bottom to save. The template then appears on the Waiver Forms screen with a Piercing category chip.
What your client sees
On a device in your studio, tap the template card on Waiver Forms, choose New Client or Return Client and hand the device over. Through a form link, the client opens the link on their own phone. Either way, they start with a PDF Preview of the waiver, then move through the steps you enabled: Client Information, Client ID Capture, Supporting Documents, Medical History, the client part of Piercing details, Custom Clauses, Media Consent and How You Found Us, finishing with a PDF Preview of what they entered. Steps that aren't switched on are skipped.
When Piercing Details is on, a form-link client taps Submit for Review rather than signing straight away, as the artist has not entered their details. The submission then waits on your Form Links screen. When your team taps Proceed, a screen lists the artist requirements: Piercing Details, and Sterilization Details and Special Instructions if you enabled them. The piercer records the session using the options from Step 3, and then notifies the client via a link to sign the final waiver (including all necessary information from the client and the artist).
Good to know
- Language is permanent. In Edit waiver the Waiver Language card shows a lock icon: "This waiver language is locked because it can only be set during creation." Create a separate template for each language you offer.
- Editing later: on Waiver Forms, tap ⋯ → Edit waiver and confirm. Your form link keeps working and shows the updated waiver.
- Skip Submission Review is only available with Piercing Details off, which also removes the piercer's session record and the data behind Piercing Overview analytics. This features allows clients to remotely sign a waiver without having staff review their submission.
